Procedures for Reserving the SPOT for Vision Screening

Any District 2-A2 Lions club that is planning a vision screening at a school, preschool, day care or community event needs to reserve a SPOT and printer.  The reservation of a SPOT is based on availability on a first come/first serve basis.  Download the SPOT Request Form and send the form to Lion Ken Book.at least two weeks prior to the event.

Once a SPOT is reserved for your event, download and print the Consent Form for Vision Screening, and the Parents Pass & Refer Letter which can be personalized for your event.  You should retain the Consent for Vision Screening form for your club records.  The Parent’s Pass & Refer Letter is to be given to the parent/guardian.  If the child/adult fails the screening, print the report on the back of the Consent Form for Vision Screening and the Parent’s Pass & Refer Letter which is to be given to the parent/guardian.

Your club will need to arrange to have one SPOT certified Lion per SPOT reserved.  Only SPOT certified Lions are authorized to operate the vision screening device.  In addition, it is recommended that you have one Lion to operate the printer and two Lions to handle registration.

Please note: If you are providing screening in a public/private/parochial school, charter school or licensed day care center where the screening results will be reported to the State, Lions need to be SPOT and State certified vision screeners.

Currently, the SPOT may be used to screen children 5 years old and younger on the date of the screening, and children with disabilities at any age for which the use of standard wall charts would not yield reliable and valid results.  If you need the support of the School Screening Program, you need to contact Lion Ken Book to schedule a team member(s) to assist you.  A two-week notice is required, and assistance is contingent on the availability of a School Screening Program team member.

After the screening is held, complete the SPOT-Final Report and email it to Lion Ken Book.
